Chronic Care Case Manager - RN
FT. Works closely with physicians, nurses, and associated clinical professionals to help patients with chronic disease obtain appropriate outpatient care and follow-up. Works to prevent potentially preventable hospitalizations. Resolves clinical, psychosocial and financial barriers on the patient's behalf. Handles difficult situations in a way that promotes the best outcomes for the patient while reducing the facility's overall cost-to-care. Has principle responsibility for identification, tracking, and navigating of chronic disease patients in the healthcare system. Plans, provides and documents patient interaction, follow-up, teaching and educates patients, family or others to attain optimal health and continuity of care. Assures that the after hospital plan of care is carried out in a safe environment through performance and supervision of subordinate personnel. Knowledge in chronic disease, specifically congestive heart failure, diabetes, chronic obstructive pulmonary disease, and bacterial pneumonia. Program development on assigned tasks. Researching, organizing, developing program material. Meeting deadlines and target goals successfully. Data collection and reporting. Graduate of an accredited School of Professional Nursing. Bachelor’s Degree Preferred. Registered Nurse currently licensed by the State of Texas. BCLS and ACLS preferred. Five years of experience as a professional Registered Nurse preferred, Program/project development experience.
Certified Diabetes Educator/Registered Dietitian
PT. Provides professional care by providing diabetic self-management education and medical nutrition therapy. The dietitian will also work closely with other departments as a liaison in order to provide for the total nutritional needs of patients. Appropriate diabetes self-management will be taught in one on one or group settings to persons with diabetes, families, or caregivers. Degree in Food and Nutrition, Dietetics, Nutritional Sciences, Human Nutrition, or comparable major. Registered Dietician with the State of Texas; Certified Diabetic Educator (CDE). Registered Dietitian currently licensed by the State of Texas. Certified by the National Certification Board for Diabetes Educators as a CDE. BCLS preferred. Experience as a professional Registered Dietitian preferred.
FT. Prepares nutritional food for hospital patients, employees and visitors. Suggests menu items and assists in the requisition of food and supplies. Operates and maintains kitchen equipment. High school graduate or equivalent preferred. One year of previous institutional food preparation experience preferred.
FT (Evening) & PRN. Performs duties involved in safely handling and disposing of blood and body fluids. Thoroughly sanitizing floors, walls, counters, and other surfaces is required, following environmental and hospital guidelines and procedures. Includes cross training in all areas and buildings. Cleaning and sanitizing furnishings and all areas assigned. High school graduate or equivalent preferred. Duties of position require extended periods of walking and standing. Lifting up to 50 pounds and transporting up to 300 pounds on carts.
FT. Nights. Responsible for the supervision of patient care activities in the Med/Surg unit. Supervises staff in the Med/Surg unit and provides oversight of operational and nursing activities. Consults with staff, physicians, Assistant Director of Nursing and the Chief Nursing Officer on nursing issues and interpretation of hospital policies to ensure patient needs are met as well as strict adherence to hospital policy. Recognizes, documents, and implements corrective action for employee behaviors outside hospital policy or standard of care under the direct supervision of Nursing Administration. Gathers, analyzes and constructs reports as required by the Chief Quality Officer. This includes the auditing of charts and records to obtain statistical data necessary to measure hospital and nursing performance. Submits reports by required deadlines. Upon data analysis completion, reports any corrective measures needed to the ADON and/or CNO for corrective action, counseling or education as needed. Responsible for the dissemination of this information to the appropriate staff. Provides professional nursing care within the Nursing Department, and coordinates care planning with other disciplines. Provides direct and indirect patient care. Provides care that reflects initiative, flexibility and responsibility indicative of professional expectation with minimum supervision. Prioritizes patient care safely, rapidly and accurately with every patient assigned to them. Determines priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system. Communicates with physicians about changes in patient's status, symptomatology and results of diagnostic studies. Responds quickly and accurately to changes in condition or response to treatment. Responsible for the scheduling of Med/Surg staff, as well as ensuring coverage for absences. Communicates with Nursing Administration briefly each day to review agenda. May be required to cover shifts as needed. This position requires on-call rotation as directed by upper level management. Med/Surg Supervisor is required to participate in educational meetings, classes and will report any new information or changes to the staff. May be required to perform emergency duty before, during and/or beyond normal work hours or days in the event of an emergency, crisis situation or disaster (man-made or natural) including evacuation sites. Graduate of an accredited School of Professional Nursing. Registered Nurse currently licensed by the State of Texas. BCLS and ACLS preferred. Two years of experience as a professional Registered Nurse in the hospital setting.
FT. Performs the primary functions of a professional nurse leader in assessing, planning, directing and evaluating patient care on a medical-surgical nursing unit. Manages all supplies and equipment and supervises all personnel and promotes team efforts with physicians and personnel of other departments. Graduate of an accredited School of Professional Nursing. Registered Nurse currently licensed by the State of Texas. BCLS and ACLS preferred. Two years of experience as a professional Registered Nurse in the hospital setting preferred.

PRN. Under the supervision of nursing staff members assists in providing nursing care, prepares records and maintains a clean environment. A student of an accredited LVN school within their last semester prior to graduation. Validation of school enrollment and status required. High school graduate or equivalent. BCLS certification preferred. One year experience in a medical setting preferred. Current CPR training. Nursing Assistant course desirable.
FT. Under supervision of a Pharmacist, assists with monitoring drug therapy, the stocking and dispensing of pharmaceuticals and the control of narcotics and controlled substances. Graduate of High School or GED. TSBP Registration, PTCB Certification (optional). One year experience in a medical setting preferred.
PRN. Under the supervision of a physician and the Director of Nursing, administers prescribed cardiopulmonary procedures, performs a variety of technical procedures and assists in the cleaning, maintenance and disinfection of cardiopulmonary equipment. Graduate of an approved JRCRTE Program. Certified as a CRTT or RRt and a Texas license required. One year experience as a Certified Respiratory Therapy Technician preferred.
FT. Works to facilitate patient flow and ensure an accurate and complete medical record for each patient. Provide nursing care to assigned patients under the supervision of a licensed professional. Graduate of an accredited School of Vocational Nursing. Licensed Vocational Nurse currently licensed by the State of Texas. One year of Licensed Vocational Nursing preferred
Business Office Clerk
FT. Schedules, greets, registers and instructs patients for appointments, prepares patient records for appointments, collects payments/co-payments for appointments. High school graduate or equivalent. One year experience in an office environment.
PRN. Assists the Program Director in the development and implementation of a high quality treatment program and the supervision of the Psychiatric Technician staff. Responsible for providing high quality patient care, displaying advanced skills in treatment planning, group treatment, individual treatment, family treatment and discharge planning. Functions as a member of the interdisciplinary team and participates in the clinic community relations program. Masters degree in Mental Health field or equivalent from an accredited psychiatric/counseling program. LCSW or LPC, and persons working toward licensure. One year post-degree experience preferred.
