Advance directives are legal documents that allow you to convey your decisions about end-of-life care ahead of time. They provide a way for you to communicate your wishes to family, friends and health care professionals, and to avoid confusion later on.

When planning for future healthcare needs, it is important to put legal documents in place to convey healthcare wishes in the event that you are unable. It is always important to discuss individual wishes with family and physicians. Completed and signed documents should be witnessed and presented to your physician, hospital and family.
